New born baby in Japan, procedure to get resident card.

As a foreigner living in Japan. You need have a resident card/passport of your baby to live in Japan. First three Important thing you need to do after you baby is born in Japan.

  • Registering the birth of your baby at the Local Ward Office or City Hall.
  • Reporting birth to your home Country's Embassy in Japan.
  • Applying for Status of Residence (visa) at the Regional Immigration Office.

Documents that you need while applying for resident card of your new born baby

At your Ward Office or City Hall.

1. Get Certificate of Acceptance of Birth Report(Shussei todoke juri shoumei sho - 出生届受理証明書): After two week when your baby born, You can add your baby address in ward office or city hall.
To add address of your baby in your address, You need Shussei Syoumensyo to get Shussei todoke juri shoumei sho. You get this, Shussei Syoumensyo from your hospital where your baby was born.
Further more you might need to submit these document(mention below) too.

  • Parent Hanko(Personal registered seal)
  • Child Health Handbook (Boshi kenko techou - 母子保健手帳),
  • Both parents' Passport and Residence Card

And then,
Shussei todoke juri shoumei sho is only then obtain after 3 days after you apply for you baby to have an address in your ward office or city hall.

2. Get Residence Certificate(Jumin-hyo - 住民票): You can either go to ward office or city hall. Or You can print out from combini store to get this Certificate.
Note: There must be name of your baby with an address.

At Your Hospital.

3. Get Medical Certificate(Iryousyoumensyo-医療証明書): Go to hospital where your baby was born, and ask for Iryousyoumensyo.

3.1 Get Child's Birth Certificate (Shussei shoumei sho - 出生証明書): You need this Certificate in your Ward Office or City Office to get Certificate of Acceptance of Birth Report(Shussei todoke juri shoumei sho)

From your company(curently working company).

4. Get Certificate of Employment(Zaishoku shoumei sho -在職証明書):

Note: If you are PR holder or Japanese Passport holder, It is mandatory. Not necessary for PR holder or Japanese Passport holder

From parent side

  • 5. Photo copy both father and mother passport.
  • 6. Photo copy both father and mother resident card.

At Embassy Of Your Country In Japan.

In my case, I am Nepali, I went to Embassy of Nepal in Japan, To make my baby passport following documents(mentioned below) were needed.

I thought, It might be better, If I apply for my baby passport too before applying resident card of your baby at immigration of Japan.

Timeline /Deadline /When to apply?

To make resident card of your baby, you must visit nearest immigration office within a one month after a baby is born, with all the documents(mentioned below) .

  • Application Form.
    To download application form from the immigration official website easily. You can get application Form k, just search in this linkclick here.
    Note: In passport number section, just mention that you have applied for the passport and are waiting for it.
  • Certificate of Acceptance of Birth Report(Shussei todoke juri shoumei sho - 出生届受理証明書)
  • Residence Certificate(Jumin-hyo - 住民票)
  • Medical Certificate(Iryousyoumensyo-医療証明書)
  • Employment Certificate(Zaishoku shoumei sho -在職証明書)
  • Photo copy both father and mother passport.
  • Photo copy both father and mother resident card.

You might need following document too. In my case, I do not submit this documents

  • Tax Payment Certificate (Nouzei shoumei sho -納税証明書 / Kazei shoumei sho -課税証明書). If both parents are working, both need to provide this document.
  • Letter of Guarantee (Mimoto hosho sho -身元保証書). This document should be signed by the head of the household. This form is provided by the immigration office.
  • Mother and Child Health Handbook (Boshi kenko techou - 母子保健手帳).

Example: if your baby was born on July 1st, Visit immigration office, complete the procedure by end July 30.

Things to keep in mind:


  • Normally, documents and certificates issued by City Hall in Japan are valid for 3 months.
  • The documents you submit for this process will not be returned to you. If you need to, keep a copy.
  • There is no need for a photo of your baby. A photo is not required for a child under 16 years old.
  • Bear in mind that your life will be quite busy around this time, and it would be wise to start getting some of these documents a month before your baby is born.
